WELL BEING & WEEKLY FOLLOW UP FORM

Each Week you will fill out the following form to provide me details on how the week went.

Important aspects covered:

  • Effectiveness of exercises,
  • Ability to perform exercises,
  • Extra coaching on movement’s needed,
  • How you feel: Energy Levels, muscular or joint pain,
  • How well you ate throughout the week

This allows me to know exactly what is going on with you week to week and to better coach you and adapt your plan.

WELL BEING AND WEEKLY FOLLOW UP FORM: THE PROCESS

Each week and after your last full workout of the week:

1. Come to this webpage, 2. Have the Trainerize app open and 3. Briefly review the workouts throughout the week to jog your memory of each exercise.

You will then go through the questions in the performance section to provide me insight on exercises you performed well, ones you had difficulty with, or any that caused discomfort.

This is important so I can adjust your plan accordingly and/or provide more coaching on specific exercises and movement patterns.

IN THE THIRD SECTION

You will give me a brief overview of healthful habits that you maintained over the course of the week. We want more good days than “bad” and more healthier choices.
